KidneyCare Oklahoma partners with physicians and care teams across the region to provide timely, specialized nephrology care. Our referral process is designed to be straightforward and reliable, supporting continuity of care while keeping communication clear and consistent for both providers and patients.

We accept referrals for the full range of kidney conditions, including chronic kidney disease, hypertension, dialysis management, and transplant evaluation.

What Happens After You Refer

Our referral workflow is structured to ensure patients are scheduled appropriately and referring providers are kept informed.

  1. Referral Received
    Referrals and supporting documentation are received via secure fax.

  2. Chart Review & Setup
    Patient records and medical information are reviewed and entered into our system.

  3. Patient Scheduling
    Patients are contacted directly to schedule their initial appointment.
    Typical scheduling timeframe: approximately one week.

  4. Clinical Evaluation
    Patients are evaluated by a KidneyCare Oklahoma provider based on the reason for referral.

  5. Communication Back to Your Office
    Office notes and clinical recommendations are sent to the referring provider after the patient’s visit.

When to Refer to a Nephrologist

Patients are commonly referred to KidneyCare Oklahoma for:

  • eGFR less than 60
  • Persistent proteinuria
  • Electrolyte imbalances, including hyponatremia and hypokalemia
  • Difficult-to-control hypertension
  • Chronic kidney disease at any stage
  • Dialysis planning or management
  • Kidney transplant evaluation or follow-up
  • Glomerulonephritis (GN)

Referrals are accepted from all medical specialties.

Specialty Services & Programs

KidneyCare Oklahoma provides comprehensive nephrology services, including:

  • Dialysis care, including coordination for in-center and home-based therapies
  • Kidney transplant coordination through the St. John Medical Center transplant program
  • Vascular access coordination in collaboration with Dr. Reddy
  • Glomerulonephritis (GN) management, led by Dr. Muthalaly and Dr. Ahmad

Care plans are coordinated across providers to support continuity and appropriate follow-up.

Communication & Follow-Up

Clear communication is a priority in every referral relationship.

  • Post-visit office notes are sent to referring providers after the patient is seen in clinic.
  • Clinical communication is transmitted through EMR interfaces when available.
